The GP Exercise Referral Scheme at Graves Tennis and Leisure Centre provides tailored exercise and activity programmes for individuals who have a medical condition where an increase
in exercise will be of great benefit to their health and well being.
The Scheme is run by a fully qualified Exercise Referral and Cardiac Rehabilitation Instructor, to ensure that the participants are prescribed the right type of activity to benefit their
health.
The Scheme is for 12 weeks and participants will be able to exercise at the centre at a reduced rate throughout the 12 week period.

Who is Eligible?
The Scheme is open to anyone who has at least ONE of the following conditions:
• Hypertension (medicated)
• Coronary Heart Disease
• Cardiac Rehabilitation Phase IV
• Diabetes
• Osteoarthritis and arthritis
• Osteoporosis
• Stroke/TIA
• Asthma
• COPD
• Depression
• Fibromyalgia
• Obesity
What we have to offer
We offer a range of exercise classes specifically designed for people on the referral programme, as well as offering the gym and swimming. We also run a Cardiac Rehabilitation Programme.
Classes
Tuesday 1.00pm-2.00pm
Wednesday 8.45am-10.15am (Cardiac Rehabilitation)
Thursday 9.00am-10.00am and 10.15am-11.15am
Friday 10.00am-11.00am
Each session is £2.50 and there are membership options at the end of the 12 weeks.
